UPPER SCREWS LOCATION AFTER FIXATION DONE 3 DAYS AGO.
Anamnesis
The patient power of the lower limbs improved,
but she is telling that the right gastrocnemius
muscle is tight. Control CT-scan of the
constructs performed the evening of
16-January-2021 showed unacceptable both upper
screws at the level of L5. The sacral screws are
acceptable. It was decided to explore the wound
redirect the upper screws.
The wound was opened and the
cross connector released. The knots were released
and the rods removed. The harvested bone was
collected to be used later. The upper screws at L5
body were flail and removed. exposure of the
transverse processes of L5. New screws were inserted
with medial trajectory to obtain proper alignment
with many checks with the C-arm. The bone graft
returned back and it was impossible to apply cross
connector. The patient was put in
Reverse Trendelenburg position with Valsalva maneuver and
hyperventilation. No CSF
leak. Routine closure of the
wound.
Smooth postoperative recovery.
She was sent to the ward.
Follow Up
The patient was discharged after performing
control CT-scan with acceptable screws position
and no sciatica and neurologically free
19-January-2021.
Comments
When the patient is complaining after
surgery, do not satisfy your self with the C-arm data. Check
with CT-scan to have better picture of the position of the
screws.
Do not discharge the patient with
unconfirmed data. If you perform surgery, do it properly.
In retrospective analysis of the event,
the cross connector was to be blamed, because it was not
articulated and during its application, it pushed the upper
screws lateral. In the future never use cross connector
without articulation.
