11-JANUARY-2005 HUSNIYEH YOUSEF RAMADAN 34 YEARS EXTRUDED DISC C6-7 WITH
INTRADURAL PENETRATION.
Anamnesis
The patient came to the clinic 05-January-2005
complaining of neck pain for 2 years with
exacerbation last 2 months with left upper limb
pain. MRI cervical spine performed recently
showing huge extruded disc C6-7 with a huge
fragment compressing the spinal cord from the
left. It seems that the fragment is penetrating
the dural sac.
On examination: The patient is in agonizing
pain. Movement of the head to all directions
associated with pain. There is weak grip,
extension left hand 3/5 and left triceps muscle
3/5.
Through anterior approach, the
level of C6-7 was identified and discectomy C6-7 was
achieved and the penetrating inside the dura were
removed. After that a CSF start to come out and a
piece of muscle was applied to the dura. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
The power of the left upper limb improved.
She was sent to the ward.
Comments
It is rare to anticipate a fragment
penetrating the dura before surgery. Taking precaution and
if the dural defect is small, covering the defect with
muscle is sufficient.
