YEARS EXTRUDED DISC L5-S1 LEFT SIDE.
Anamnesis
The patient came 11-January-2004 complaining of LBP for 2 months with
left
sciatica for 2 weeks. MRI
performed 05-January-2004 showed huge extrusion L5-S1 with
left downward migration.
On examination: He was limping with
exaggerated scoliotic stance with weak dorsiflexion both feet. SLRS
left side was 85 degrees with pain. There is hypalgesia left S1 root
territory.
The patient
underwent left L5-S1 hemiflavotomy with foraminotomy of the
left S1 root. The extrusion was removed from under the axilla.
Meticulous cleaning of the disc space was performed and root became
free. The patient was put in Reverse Trendelenburg position with
Valsalva maneuver and hyperventilation. No CSF
leak. A free fat was harvested from the
subcutaneous layer and covered the exposed parts
of the root. to minimize postoperative scarring. Routine closure of the wound.
Improvement of the power and the patient was discharged the next
day.
FOLLOW UP
The patient came to the clinic 21-February-2004
with clean wound and SLRS 80 degrees left side
with pain and no motor, nor sensory deficit.
Comments
The estimated postoperative recurrence is still
ranking around 7%, because the disc space is
still relatively not shallow.
