45 YEARS MULTIPLE INJURIES WITH FRACTURE D6-7 WITH PARAPLEGIA.
Anamnesis
The patient admitted to the ICU of Shmaisani
hospital 06-October-2004 after falling down with
severe head injury, fracture frontal bone left
side, fracture D6-7 with paraplegia below this
level, right pneumothorax, fracture ribs,
fracture left femur, fracture both bones both
legs and left clavicle.
On examination, the patient
beside the urgent measures performed to his
pneumothorax and orthopedic manipulations, in
Foley's catheter with complete paraplegia below
D5. GCS became 15/15 and his vital signs
stabilized was taken to the operating room.
Skeletonization of D4 down to
D9 was performed. The spinal cord was was cut
and out of the dura. The dura was repaired and
using Hartchell rectangle with eight sublaminar
screws 4 above and four below fixation of the
spinal column was achieved. Routine closure and
the patient was sent back to the ICU.
FOLLOW UP
The patient
continued treatment in the ICU and from there he was
discharged.
Comments
The aim of the
surgery is early mobilization. The prognosis of
the paraplegia is gloomy and the multiorgan
failure, still threatening his life.
