YEARS HUGE EXTRUDED DISC C3-4 WITH SPINAL CORD COMPRESSION.
Anamnesis
The patient came to the clinic 20-September-2003
complaining of neck pain for 3 months when
looking up with numbness of the right side of
the body. He felt down 2 years ago after what he
noticed his complains which progressed over
time. MRI cervical spine done 21-August-2003
showing severe cervical canal stenosis C3-4 with
malacia of the spinal cord at this level with
compression.
On examination: the patient limping with wide based legs gait. He has pain when
looking up with weak grip
and extension right hand 3/5 and right triceps
3/5. Hoffmann positive right side and all
deep reflexes are exaggerated right side. There
is weak dorsiflexion right foot.
Trans-nasal intubation. Discectomy of C3-4
was done until the dura was seen along all the
posterior aspects. All stages of surgery were
done under C-arm control.
Routine closure of the wound.
Smooth postoperative recovery. The power of the
right upper limb became normal. H e was sent to
the ward.
FOLLOW UP
The patient
came to the clinic 05-October-2003 with clean wound
and the power of the right upper limb improved and
numbness decreased.
The patient
then came 19-July-2004 complaining of numbness
right hand with Hoffman sign disappeared and he is
neurologically free.
Comments
The recurrence rate in
cervical disc surgery is zero.
