16-JULY-2003 IDREES MUHAMED AL-DAMEN 40 YEARS
EXTRUDED DISC L5-S1 WITH RIGHT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 10-July-2003
complaining of LBP with right sciatica for 8
months with numbness right S1 territory.
On examination, the patient is limping with
exaggerated scoliotic stance. SLRS was 30
degrees with pain right side and 45 degrees with
less pain in the left. There is weak dorsi and
planterflexion right foot 4/5.
The patient was sent for investigation and MRI
of the lumbar spine showed extruded disc L5-S1
with right downward migration.
Right L5-S1 hemiflavotomy
with foraminotomy right S1 root. The extruded
disc was removed lateral to the axilla and right
sided intradiscal cleaning L5-S1 disc space was
achieved.
Fat transfer was performed to minimize
postoperative adhesions. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak. Routine closure of the wound.
Smooth postoperative recovery.
The power of right foot normalized. He was sent to the ward.
FOLLOW UP
The patient
came to the clinic 23-July- 2005 and 16-March-2008 complaining
of right hip pain, but SLRS was 80 degrees both
sides with no sensory or motor deficit and at both
visits he was asked to performed check MRI and given
medication, but he never did investigations and
disappeared.
Comments
The estimated postoperative
recurrence is around 7%, because the disc space is
not shallow.

its sensor gives feed back about the localization.
