RESIDUAL AFTER GUN SHOT BOTH FEMURS WITH DROP BOTH FEET AND COMPLETE
NONFUNCTIONING LEFT PERONEAL NERVE.
Anamnesis
The patient a Palestinian citizen came to the clinic
30-January-2003 telling that he got gun shoot by
Israelis' militants 03-September-2002 after what
he progressed bilateral drop feet.
On examination: the patient has complete non
function both peroneal nerves with drop both
feet and loss of sensation. X-rays showing
healed fracture both lower third of the femurs
with shrapnel.
The patient was resent for ECS both sciatic
nerves and confirmed complete non- function of
the left peroneal nerve.
The left sciatic nerve was
exposed and the dissection followed down to the left
peroneal nerve. The tibial division is normal, and
direct stimulation of the left peroneal nerve after
neurolysis showed some signs of conductivity for 8
Volts. exploration for neuroma was negative. It was
decided to satisfy with neurolysis. Routine closure of
the wound
Smooth postoperative recovery.
The neurologic status the same. He was sent to the ward.
Follow Up
The patient came to the clinic 16-February-2003
with clean wound and telling that sensation of
the left L5 territory improved and and there is
some contraction of the peroneal nerve group of
muscles. He was advised to be followed by
rehabilitation, where he lives.
Comments
The peroneal nerve has bad reputation in
recovery, especially when suffer such diffuse damage.
