12-JUNE-2004 IMAD MUHAMED SANAD 39 YEARS
EXTRUDED DISC L4-5 WITH SEGMENTAL STENOSIS.
Anamnesis
The patient came to the
clinic 24-May-2004 complaining of LBP with
right sciatica for 6 years with exacerbation
last week down to S1 root
territory. MRI lumbar spine
done 30-March-2004 showing huge extruded disc
L5-S1 with right downward migration and extruded
disc L4-5.
On examination, the patient in agonizing pain,
limping with exaggerated scoliotic stance with
weak dorsiflexion right foot 4/5 with hypalgesia
right L5 and S1 territories. SLRS was 40
degrees with pain both sides.
The patient was sent for
investigations and MRI lumbar spine done
01-June-2004 showing huge extruded disc L4-5
with segmental stenosis and small old extrusion
L5-S1.
Decompressive laminectomy
L4-5 with foraminotomy both L5 roots. Bilateral
removal of the extrusion and bilateral
intradiscal cleaning L4-5 disc space.
The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. A free fat was
harvested from the
subcutaneous layer and covered the exposed parts
of the dura and roots to minimize postoperative
scarring. Routine closure
of the wound.
Smooth postoperative recovery. The power of
the right foot normalized.
FOLLOW UP
The patient
came to the clinic 05-July-2004 with clean wound and no motor,
nor sensory deficit. SLRS was 80 degrees both sides
without pain.
Comments
There is still an
estimated postoperative recurrence around 7%,
because the disc space is still not completely
collapsed.
