The patient came to the clinic 03-June-2006 complaining of
LBP for 4 years with right sciatica for 3 months with
exacerbation last 8 days. MRI performed 27-May-2006 showing huge extrusion of
L4-5 with right downward migration. On examination: SLRS was 60
degrees in the right and 50 degrees in the left with weak
dorsiflexion both feet more the right and hypalgesia right L5
and S1 roots territories. He was
advised to undergo surgery and given pain-killers.
Bilateral L4-5 flavotomy with right L5 root foraminotomy was done
and the extruded disc was removed in one piece from under the
axilla in one huge piece. Minimal cleaning of the disc space was applied
lateral to the axilla.
Routine closure of the wound. Smooth postoperative neurological
recovery. |
