16-MAY-2007 IMAD MUHAMED AHMAD THIYAB 29 YEARS EXTRUDED DISC L4-5 WITH LEFT
FORAMINAL COMPRESSION.
Anamnesis
The patient came to the clinic 13-May-2007
complaining of left sciatica for 40 days,
progressing in frequency.
On examination: the patient had scoliotic stance
with painful SLRS 20 degrees in the left . There
is almost drop left foot, with hypalgesia left
L5 and S1 territories.
MRI lumbar spine performed 05-May-2007 showing
bulging disc L5-S1 with extruded disc L4-5 with
left foraminal compression.
Left L4-5 hemiflavotomy with foraminotomy of
left L5 root was performed. The extruded disc
was removed lateral to the axilla. Meticulous
intradiscal cleaning from the left side. The
disc material was friable and inflammatory in
appearance, confirming that discitis having
place as the cause of his pathology. Gentamicin
was flushed to the intradiscal space and routine
closure of the wound.
Smooth postoperative recovery.
Comments
The disc material was very friable and
inflammatory in nature, indicating that localized discitis
having place. It could be the cause of his extrusion and
clinical course.
Meticulous cleaning of the disc space and
flushing the space with antibiotic is mandatory. The patient
must be kept in antibiotic cover for a month at least to
prevent spread of the infectious process.
