Anamnesis
The patient came to the emergency of Shmaisani
hospital 24-January-2005 with agonizing LBP and
bilateral sciatica more the right for less than
a week. The patient has bronchial asthma for 3
years without medications.
On examination: The patient is in agonizing
pain, limping with exaggerated scoliotic stance.
There is drop right foot ,
planterflexion same foot 3/5. SLRS was 10 degrees with pain
right
side and 45 degrees left side with less pain. There is hypalgesia
right L5, S1
territories.
The patient was sent for investigations and MRI
lumbar spine showed a huge extruded disc L4-5
Extended right L5 foraminotomy
with removal of the huge extrusion and right sided
intradiscal cleaning of L4-5. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
The power of the right foot improved.
He was sent to the ward.
Follow Up
The patient came to the clinic 09-February-2005
with normalization of the power of the right
foot and decrease hypalgesia of the right L5,S1
territory. SLRS was 45 degrees both sides
without pain.
Comments
There is still an estimated postoperative
disc recurrence around 7%, since the disc space is not
completely shallow.
