Comments
In retrospective analysis, Harrington
rods fixation is an easy task, but it has cons and pros. It
is not MRI compatible. It needs 2 levels above and 2 levels
below the the fracture site. Disruption of the interspinous
ligament as in this case, prevent proper distraction.
Transpedicular fixation is superior to
this technique.

and at this time there are a lot of options, you can choose
the best for the proper situation.
Anamnesis
The patient after falling down 2 days ago got
LBP with inability to walk, and she was admitted
to Shmaisani hospital. The patient is
neurologically free, but X-rays, CT-scan of the
spine and MRI showing unstable crushed D12
On examination: The patient in agonizing pain in
bed with Foley's catheter , but neurologically
free.
Skeletonization of D10 down
to L2, trying during that preserve the
interspinous ligaments, which was torn between
D12-L1 and the left lateral mass was destroyed.
Using Harrington distraction fixation rods,
fusion of L2 up to D10 was applied with control
for correction of the crushed body of D12, until
it got acceptable shape. Due to disruption of
the interspinous ligament a sublaminar wires
were added at D11 to provide more stability of
the construct. No CSF leak. Routine closure of the
wound.
Smooth postoperative recovery.
She was sent to the ward and ambulated the next
day.
Follow Up
The patient came to the clinic 29-April-2002
with clean wound walking and neurologically
free.
