The patient started to complain of gradual onset of inability to walk for several months with numbness four limbs for several years. She is a known diabetic with hypertension. On examination she had weak four limbs more in the right side. MRI of the cervical region showed severe stenosis with OPLL  and MRI of the lumbar spine also showed stenosis. Anterior decompression with removal of the compressing OPLL was achieved and cage fixation was performed to all mentioned levels.
