30-APRIL-2015 INTISAR MUHAMED AL-SHQUR
45 YEARS EXTRUDED DISC L4-5 WITH RIGHT EXTRAFORAMINAL AND LEFT
INTRAFORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic with a history of
LBP and left sciatica 1 year ago for 2 months,
then right sciatica for 2 weeks shooting down to
the big toe right foot. MRI lumbar spine done
04-June-2013 bad quality showing as having
retrolisthesis L3-4.
On examination; the patient is limping with
exaggerated scoliotic stance. Neck and upper
limbs were neurologically free. SLRS was 80 degrees with pain in the
right. There is weak dorsiflexion
right foot -4/5.
The patient was sent for investigations and MRI
lumbar spine performed 28-April-2015 showing
severe lumbar canal stenosis L4-5 with right
huge extraforaminal extrusion and left foraminal
occlusion. Dynamic studies ruled out
overmobility.
Using the C-arm, the L4-5 level was identified.
Foraminotomy both L5 roots. The right
extraforaminal extrusion was followed and
removed in several pieces and check for
extraforaminal remnants was negative. Bilateral
cleaning L4-5 disc space. Routine closure of the wound.
Smooth postoperative recovery. The power of both
feet became normal.
Comments
The patient still having an estimated
postoperative recurrence around 7%.
