The patient came to the clinic 24-April-2006 with LBP without
sciatica. On examination, he had scoliotic stance with SLRS 45
degrees in the right and weak dorsi and planterflexion right foot.
The patient was sent for MRI and the
MRI showed huge extrusion of L4-5 disc to the right with elements of
segmental stenosis at that level with bulging L5-S1. . The patient
was advised to undergo surgery.
Bilateral L4-5 flavotomy was done with removal of all compressing
elements from both sides, including a ganglion from the right facet
joints. Right sided cleaning of L4-5 disc space, which showed some
elements of inflammatory process. Foraminotomy of right L5 root was
performed. The right L5 root was swollen.
Dramatic postoperative recovery and the patient was discharged
the next day. |
