YEARS EXTRUDED DISC L4-5, MORE TO THE RIGHT AFTER DECOMPRESSIVE
LAMINECTOMY L4-5 AND L5-S1.
Anamnesis
The patient came
to the clinic 19-January-2002 complaining of LBP
with bilateral sciatica 2 months after performed
to him decompressive laminectomy 1996 for LCS
L4-5 and L5-S1. The patient walking with
crutches.
On examination: The patient
is limping with exaggerated scoliotic stance
with SLRS was 15 degrees with pain right side and
weak dorsi and planterflexion right foot -4/5
and hypalgesia right L5 root territory.
The patient sent for investigations and MRI
lumbar spine performed 27-January-2002 showing
extruded disc L4-5 more to the right.
Foraminotomy right L5 roots after massive scar
and neurolysis. The disc of L4-5 was removed from
the right side of the axilla and meticulous cleaning
of L4-5 disc space was achieved. The patient was put in Reverse
Trendelenburg position with Valsalva maneuver
and hyperventilation. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
The power of the right foot improved.
He was sent to the ward.
Follow Up
The patient the
came to the clinic 14-February-2002 with
clean wound with no sensory nor motor deficit
and SLRS 80 degrees without pain in the right.
