HUGE EXTRUDED DISC L4-5 MORE O THE RIGHT.
Anamnesis
The patient came to the clinic 07-June-2004
complaining of LBP with right sciatica for 3 months.
On examination, the patient in agonizing pain,
limping with exaggerated scoliotic stance with
weak dorsiflexion right foot 4/5. SLRS was 70
degrees with pain right side with 45 degrees
left side shooting to the right leg. There is
hypalgesia right L5 root.
The patient was sent for
investigations and MRI lumbar spine done
08-June-2004 showing huge extruded disc L4-5
more to the right.
Foraminotomy right L5 root.
Right sided removal of the extrusion and
right sided
intradiscal cleaning L4-5 disc space.
The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. A free fat was
harvested from the
subcutaneous layer and covered the exposed parts
of the dura and roots to minimize postoperative
scarring. Routine closure
of the wound.
Smooth postoperative recovery. The power of
the right foot normalized.
FOLLOW UP
The patient
came to the clinic 26-June-2004 with clean wound and no motor,
nor sensory deficit. SLRS was 90 degrees both sides
without pain.
The patient
came several times over the years then came
09-March-2013 with MRI lumbar spine performed the
same day showing recurrence of the disc L4-5 . The
patient was given medications and in case of not
improving to consider surgery and he disappear.
Comments
There is still an
estimated postoperative recurrence around 7%,
because the disc space is still not completely
collapsed.
