HUGE WIDE BASED EXTRUDED DISC C5-6 WITH MALACIA OF SPINAL CORD AND EXTRUDED DISC
C6-7.
Anamnesis
The patient came to the clinic 18-July-2017
complaining of neck and left upper limb pain for 18 months with
exacerbation last 5 days. MRI cervical spine done
04-January-2017 showing huge wide based extruded
disc C5-6 with malacia of the spinal cord and
extruded disc central at C6-7.
On examination, the patient is in agonizing
pain, with pain triggered when turning the head
to the right shoulder and flexing the head. Grip
of the right hand 4/5. left hand -4/5, extension
of the right hand 4/5, the left hand 3/4. Both
triceps 4/5. He has weak dorsiflexion left foot
and the left quadriceps femoris 4/5.
MRI
repeated 30-July-2017 showing the same previous
extrusions.
Discectomy of C5-6 with
removal of the wide based extrusion until the
dura was seen posterior to the posterior edge of
C5 and C6 bodies. Discectomy of C6-7 with
removal of the central extrusion. 2 cages
ON PLUS (EOS CERVICAL BLADED CAGES 13X17 N6 were
inserted to the disc spaces and fixed. All
stages of surgery were guided with C-arm.
Routine closure of the
wound.
Smooth postoperative recovery. The power of
both upper limbs normalized and he was sent to the ward.
Comments
The patient has radiculo-myelopathic
syndrome due to spinal cord compression. Surgical
decompression and insertion of bladed cages was a necessity.
Skyra MRI with all clinical applications in the run since 28-Novemeber-2013.
Inomed Riechert-Mundinger System, with three point
fixation is the most accurate system in the market. The microdrive and
its sensor gives feed back about the localization.
