CRUSHED D8 AND D9 AFTER FALLING DOWN.
Anamnesis
The patient was transferred from other hospital
after falling down at the morning without loss
of consciousness. CT-scan of the brain was
normal and CT-scan of the spine showed crushed
fracture of D8 and D9 with fracture of the
laminae of subsequent vertebrae. MRI performed showing
compression at the fracture site with edema of
the spinal cord.
On examination: the patient is neurologically
free with ecchymosis of the right eye and the
forehead.
Skeletonization of D6,7,8,9,10
and 11 was achieved with preservation of the
ligaments between the posterior processes. The left
D7-8 facet is fractured but acceptable for fixation.
Using Xia 4 transpedicular screws at the level of
D6,7,10,11 reduction and fixation of the spine was
achieved. Screws all monoaxial 5.5mm X40 mm to D10
and D11. Screws 5.5mm x 35 mm to D7,6. Before inserting the rods, motor
stimulation of the screws with 8 V did not show any
response. The left screw at D10 level showed
CSF leak, which was explained by the injury, but it
was stopped after inserting the screw. The fractured
bones got acceptable alignment after that. Routine closure of the
wound. The patient was given 1 unit blood the
next day after surgery.
Smooth postoperative recovery. He was sent to the ward.
Comments
The fracture is unstable and compressing
the swollen spinal cord. Distraction-reduction of the
fracture with subsequent stabilization is a must, to prevent
further spinal cord damage.
