23-JANUARY-2005 ISMAEEL MUHAMED SHAWEESH 36 YEARS EXTRUDED DISC L4-5 LEFT SIDE.
Anamnesis
The patient a Libyan citizen came to the
emergency of Shmaisani hospital with severe LBP with left
sciatica. MRI lumbar spine performed
showing extruded disc L4-5
with left foraminal occlusion. The patient was
operated 10-May-2004 in Singapore for PLD L5-S1.
On examination: The patient is in agonizing
pain, limping with exaggerated scoliotic stance.
There is weak dorsiflexion left foot -4/5,
planterflexion same foot 4/5. SLRS was 10 degrees with pain left
side. There is hypalgesia left L5, S1
territories.
Extended left L5 foraminotomy
with removal of the huge extrusion and left sided
intradiscal cleaning of L4-5. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
The power of the left foot improved.
He was sent to the ward.
Follow Up
The patient came 02-February-2005 with no
sciatica and no motor, nor sensory deficit.
The last visit 28-July-2012 came complaining of
LBP for 3 days. MRI lumbar spine performed
27-July-2012 showing no recurrence and dynamic
studies ruling out overmobility.
Comments
There is still an estimated postoperative
disc recurrence around 7%, since the disc space is not
completely shallow.
