LCS L3-4, L4-5 WITH EXTRUDED DISC L5-S1 MORE TO THE LEFT.
Anamnesis
The patient is a doctor, came to the clinic
19-August-2014 complaining of LBP for 27 years
with left sciatica for 2 years with drop left
foot for 6 months. MRI lumbar spine done
16-June-2014 showing severe lumbar canal
stenosis L3-4, L4-5 and old extruded disc L5-S1
more to the left.
On examination; the patient is limping as
dragging the left leg with exaggerated scoliotic
stance. SLRS was 80 degrees both sides with pain
.Dorsiflexion right foot -3/5, left complete
drop foot. Planterflexion both feet 4/5. The
patient was sent for new investigations and done
confirming the previous data.
Skeletonization of L3,4,5 and upper sacrum. The
segments were stable, even the L5 was fused with
S1. Decompressive laminectomy L4 and L5 and
lower half of L3 and upper sacrum. Foraminotomy
left L5 root and both S1 roots. Inspection of
the calcified L5-S1 annulus fibrosis. It was
decided not to violate the disc space. The left
L5 and S1 roots were severely damaged by the old
compression. There was no need for fusion.
Routine
closure of the wound.
Smooth postoperative recovery. The power of the
right foot and planterflexion left foot became
normal and the drop left foot became better. He
can dorsiflex the big toe but still weak 3/5.
Comments
The patient has old lumbar canal stenosis
and old extrusion with old drop left foot. Time will show
the effect of the decompression.
