30-JUNE-2004 IYAD MUSTAFA JIAARAH 31 YEARS
EXTRUDED DISC L4-5 AND L5-S1.
Anamnesis
The patient came to the clinic 28-June-2004
complaining of LBP with bilateral sciatica, more
the right for 1 month. MRI lumbar spine
performed 30-May-2004 showing huge extrusion
L4-5 and L5-S1 more to the right.
On examination, the patient in agonizing pain,
limping with exaggerated scoliotic stance with
weak dorsi and planterflexion right foot 3/5. SLRS was
30
degrees with pain right side and 75 degrees left side
with pain.
Laminectomy L5 and lower third
of L4. Foraminotomy both L5 roots and both S1
roots. Right sided removal of the extrusion and
right sided
intradiscal cleaning L4-5 and L5-S1 disc spaces.
The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. A fat graft with
pedicle was
achieved from the
subcutaneous layer and covered the exposed parts
of the dura and roots to minimize postoperative
scarring. Routine closure
of the wound.
Smooth postoperative recovery. The power of
the left foot normalized.
FOLLOW UP
The patient
came to the clinic 10-July-2004 with clean wound and no motor,
nor sensory deficit. SLRS was 70 degrees both sides
with pain.
The patient
came several times and the last visit was
18-May-2009 telling that he got LBP for one month
after working as driver in taxi. He was
neurologically free and given medications.
Comments
There is still an
estimated postoperative recurrence around 14%,
because the disci space is still not completely
collapsed.

its sensor gives feed back about the localization.
