YEARS OLD EXTRUDED DISC L4-5 WITH RIGHT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic
19-August-2003 complaining of LBP for one year with
right sciatica to the right L5 territory. MRI
lumbar spine done 14-August-2003 showing old extruded disc
L4-5 with right foraminal occlusion.
On examination, the patient is limping with
exaggerated scoliotic stance. SLRS was 80
degrees right side with pain. There
is weak dorsi and planterflexion right foot 3/5.
Right L4-5 hemiflavotomy with
foraminotomy right L5 root. The extruded disc was
removed lateral to the right axilla using
drilling and right
sided intradiscal cleaning L4-5 was achieved.
The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. Routine closure of the wound.
Smooth postoperative recovery.
The patient showed improvement of the power of
his right foot.
He was sent to the ward.
FOLLOW UP
The patient
came to the clinic 31-August-2003 with clean wound.
SLRS was 85 degrees without pain right side with no
motor nor sensory deficit.
Comments
There is still an estimated postoperative
recurrence less than 7%, because the disc space
is shallow, but not completely collapsed.
