23-AUGUST-2007 JEHAD MUHAMED AL-KHALAFAT 40 YEARS HUGE PLD L5-S1 WITH RIGHT
DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 16-December-2006
complaining of LBP for 1 month with right
sciatica and he was claiming, that he was
improving.
On examination SLRS was 80 degrees both sides
with no neurologic deficit.
MRI lumbar spine was performed at that time and
showed small extrusion L5-S1 right side with
bulge disc L4-5. The patient was recommended to
undergo conservative treatment.
The patient came with agonizing pain for several
days and was hospitalized and repeat MRI
performed 22-August-2007 showed massive
extrusion L5-S1 and the patient was urging for
surgery.
L5-S1 right 2/3 hemiflavotomy was performed and
the huge extrusion was seen from under the
axilla of the right S1 root pushing the root
lateral. It was removed in several pieces and
meticulous cleaning of L5-S1 was performed from
the right. The epidural fat was preserved during
surgery to prevent postoperative fibrosis.
Routine closure and smooth postoperative
recovery and normalization of the power of the
right foot.
Comments
The patient had a previous disc, which was
not convincing for surgery. When the extrusion came more
pronounced, the clinical picture of the patient dramatically
deteriorated and surgery was the only solution for his
condition.
Suction sometimes remove the epidural fat during surgery. This
must be avoided to preserve the fat tissue to prevent subsequent
postoperative fibrosis.
