The patient was operated for PLD L5-S1 1990 for right sciatica. 16-April-2005 got left sciatica. On examination, she had weak planterflexion all toes left foot with SLRS left side 75 degrees. MRI done 30-June-2005 showing extruded disc L4-5 left side with downward migration. The patient was operated through left hemiflavotomy with foraminotomy of the left L5 root . Minimal cleaning of the disc space was performed after removal of the extrusion.
