11-FEBRUARY-2015 KAMAL FAWZI ANBA 48 YEARS
HUGE EXTRUDED DISC L4-5 WITH RIGHT DOWNWARD MIGRATION WITH SEGMENTAL STENOSIS.
Anamnesis
The patient came to the clinic 09-February-2015
complaining of right sciatica for 10 days with
numbness right foot. 1 year ago had minor
episode in the left side and disappeared after
10 days.
On examination, the patient is in agonizing pain
limping with exaggerated scoliotic stance with
SLRS 30 degrees with pain in the right and
slight decrease of the right AJ and weak
dorsiflexion right foot 3/5 and left foot 4/5
with hypalgesia right S1 territory.
MRI lumbar spine done 10-February-2015 showing
huge extruded disc L4-5 with right downward
migration causing segmental stenosis.
Using the C-arm, the level of L4-5 was
identified. Partial laminectomy lower 2/3
of L4 and upper 1/5th of L5. Foraminotomy both
L5 roots. The extruded disc was attacked from
the right and the downward migrating big piece
was removed. Bilateral cleaning L4-5 disc space.
The disc space was filled with Omnipaque and
control image was achieved. It seems that even
with meticulous cleaning the disc space is still
have plenty filaments from the endplates. Routine closure of the wound.
Smooth postoperative recovery.
The power of the both feet became normal
immediately after surgery.
Comments
The patient still has an estimated
postoperative recurrence around 7%.
