HUGE WIDE BASED EXTRUDED DISC L4-5 MORE TO THE LEFT.
Anamnesis
The patient came to the clinic 01-October-2015
complaining of LBP for 4 months and left
sciatica for 1 month down to the left knee . He
improved then deteriorated the last week.
On examination: He is limping with exaggerated scoliotic
stance with weak dorsiflexion left foot 3/5 and
planterflexion same foot 4/5. SLRS was 20
degrees right side with pain shooting to the
left limbs and 15 degrees with more pain in the
left side.
The patient was sent for investigations. MRI lumbar spine done
01-October-2015 showed extruded disc L4-5, wide
based more to the left.
Left L5 root foraminotomy
with removal of the left upward migration extrusion lateral to the
root which was shifted medially. Left sided
intradiscal cleaning L4-5 disc space.
Subaxillary inspection for extrusion remnants.
The root is lax and free. Using MultiGen a
pulsed mode RF with 42 degrees Celsius, 240 sec,
2 Hz and 20 millisecond pulse width the left L5
root was manipulated using 2 curved catheters
above and below the axilla. Routine closure of the
wound.
Smooth postoperative recovery. The power of
the left foot became normal. No sciatica. The patient was
sent to the ward.

Comments
The patient still has an estimated
postoperative recurrence around 7%.
This is the 11th case during which PRF was
used. It seems that the sciatica disappearing better in
comparison with patients not exposed to such manipulation.
