04-SEPTEMBER-2015 KHALED MISLEH AL-RASH 37 YEARS
HUGE EXTRUDED DISC L4-5 WITH RIGHT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 03-September-2015
complaining of LBP for 7 months and right
sciatica for 1 month with numbness all toes and
feeling of coldness of the right foot. MRI
lumbar spine done 29-August-2015 showing huge
extruded disc L4-5 with right downward
migration.
On examination: the patient was in agonizing
pain, using crutches for walking for 2 days,
limping with exaggerated scoliotic stance. SLRS
was 20 degrees with pain in the right and 30
degrees with pain shooting to the right leg. The
right KJ is absent in the right. There is weak
dorsiflexion right foot -2/5 and planterflexion
same foot 3/5. There is analgesia right L5
territory and hypalgesia right S1 territory.
Foraminotomy right L 5 root
with subaxillary removal of the huge extrusion
and right sided cleaning L4-5 disc space.
Using pulsed mode with parallel mode and curved
catheters, a current with 2 Hz 20 ms duration
and 42 Celsius for 240 sec was applied to the
right L5 root. Routine closure of the
wound.
Smooth postoperative recovery. The power of
right foot became normal with no sciatica, nor
numbness of the foot. The patient was
sent to the ward.
Comments
The patient still has an estimated
postoperative recurrence around 7%.
This is the third case using PRF with
MultiGen. There is a sense that the neuropathic pain is
disappearing in the postoperative course after disk surgery.
It is still early to judge.
