05-OCTOBER-2014 KHALED UTHMAN AL-SIBAEE 72 YEARS
HUGE EXTRUDED DISC C3-4 WITH SPINAL CORD COMPRESSION AND TETRAPARESIS.
Anamnesis
The patient came to clinic 02-October-2014
complaining of neck pain for 2 months with
inability to walk and numb four limbs for 1
month. The patient is a known hypertensive for 4
years. MRI cervical spine done 30-September-2014
showing huge extruded disc C3-4 with spinal cord
compression and malacia.
On examination; the patient in wheelchair. There is neck pain
when turning the head up and to the right and
bending it to both shoulders. The right deltoid
and both biceps are 4/5. Grip both hands 3/5 and
extension both hands 2/5 weaker in the right.
The triceps right-3/5 and the left 4/5. There is
hypalgesia from the right C4 and below to
include the right lower limb. The deep reflexes
are high both sides with Hoffmann sign positive
both sides. There is weak both lower limbs -3/5
all muscles more weak in the left side with
Babinski positive in the left side.
Discectomy C3-4 with removal of the all
extrusion until the dura was seen behind.
Vitatech cervical cage 3x16x6.5 mm inserted. Using
cervical plate one level 26 mm with 2
fixed 4x16 mm screws to C3 and 2 screws revision
4.5x16 mm to C4, fusion of C3-4 achieved.
Routine closure of
the wound.
Smooth postoperative recovery. The power of
all four limbs became normal except the left
foot dorsiflexion which improved to +3/5.
Comments
The patient has radiculo-myelopathic
syndrome, which functionally improved after decompression.
