EXTRUDED DISC L4-5 WITH RIGHT UPWARD MIGRATION.
Anamnesis
The patient came to the clinic 03-November-2015
complaining of LBP and right sciatica for 3 days
down to the lateral malleolus with positive
cough sign. The patient is a known diabetic for
4 months and with sarcoidosis with last attack
1997. He is under treatment with prednisolone 5
mg once daily until now.
On examination: the patient was limping with
exaggerated scoliotic stance. SLRS was 15 degrees
with pain right side. Weak dorsiflexion
right foot 4/5.
The patient was sent for investigations and MRI
done 04-November-2015 showed huge extruded disc
L4-5 with right upward migration. Dynamic
studies showed hint of mild spondylolisthesis
L4-5, which could be due to impaction of the
extruded disc right upward.
Foraminotomy right L5 root
with extension of the exposure far upward.
The right L4 root was severely compressed by the
extrusion The extrusion was removed lateral to
the axilla of L5 root.
Right sided intradiscal cleaning of L4-5 disc
space. Using MultiGen, a pulsed mode
RF with 42 Celsius, 240 sec, 2 Hz and 20 mesec
duration the right L4 root was achieved.
Routine closure of the
wound.
Smooth postoperative recovery. The power of
the right foot became normal without any sciatica. The patient was
sent to the ward.

Comments
The patient still has an estimated
postoperative recurrence around 7%.
This is the 16th case using the PRF mode
with MultiGen.
