Anamnesis
 |
The patient came to the clinic
22-September-2002
complaining of LBP with right sciatica for 5 months
with numbness right S1 territory. MRI
lumbar spine done 15-August-2002 showing huge
extruded disc L5-S1 right side. |
|
|
On examination: The patient in agonizing pain,
limping with exaggerated scoliotic stance.
SLRS
25 degrees
with pain in the right and 85 degrees in the
left without pain. There is severe weak dorsi planterflexion
right foot 3/5, with hypalgesia right
L5 and S1 territories. |
|
|
Right S1 foraminotomy with removal of the
extrusion lateral to the axilla from the
right. Right sided intradiscal cleaning L5-S1.
The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak.
Routine closure of the wound. |

|
Smooth postoperative recovery.
The power of the right foot improved.
He was sent to the ward. |
Follow Up
|
|
The patient came to the clinic 03-October-2002
with clean wound and still having weak
dorsiflexion right foot 4/5 with
SLRS 50 degrees without pain right side. |

Comments

|
There is still an estimated postoperative
recurrence around 7%, because the disc space is still not
shallow. |
|
