30-JANUARY-2015 KHALED ABDALLAH AL-RANTISI 51 YEARS
EXTRUDED DISC L4-5 WITH SEVERE COMPRESSION OF THE LEFT L5 ROOT.
Anamnesis
The patient came to the clinic 26-January-2015
complaining of LBP for 12 years with left sciatica
for 30 days with numbness and pain all toes left foot. MRI
lumbar spine done 05-January-2015 showing
bulge L3-4, L4-5 and extruded disc L5-S1 more to
the left. The patient had episode of suspected
MI and cath done 1 week ago and was normal. He
was given Isokit 5 mg twice a day, baby aspirin
and Lipitor 20 mg .
On examination, the patient is in agonizing pain, limping with exaggerated scoliotic
stance. SLRS was 70 degrees with pain in the
left. There is weak dorsiflexion left foot
4/5.
The patient was sent for new full
investigations. MRI lumbar spine done
27-January-2015 showing extruded disc L4-5 with
severe compression of the left L5 root with
bulge L5-S1. Dynamic studies were normal and
cardio consultation was acceptable.
Using C-arm the level of L4-5 was identified and
foraminotomy left L5 root was performed. The
extruded disc was removed lateral to the axilla
and the downward migrating piece was removed and
left sided intradiscal cleaning was done.
Routine closure of the wound.
Smooth postoperative recovery. The power of the
left foot became normal.
Comments
There is still an estimated postoperative
recurrence around 7%.
