5-6 WITH SEVERE SPINAL COMPRESSION AND MALACIA OF THE SPINAL CORD.
Anamnesis
The patient came to the clinic 02-September-2018
complaining of neck pain for 5 months with
numbness four limbs the last month. MRI cervical
spine performed 02-September-2018 bad quality
showing huge extruded disc C4-5 with severe spinal
cord compression and malacia of the spinal cord
with bulge C5-6. The patient has allergy to
Voltaren group.
The patient on examination, had ataxic gait with neck pain when looking up and
to the right. There was weak grip right hand -4/5, extension of
both hands -3/5, right triceps
3/5, left triceps 4/5 with weak dorsiflexion
both feet 4/5. The patient disappeared and came
22-January-2019 urging for surgery and he was sent
for investigation.
MRI cervical performed 22-January-2019 showing
huge extruded disc C4-5 and C5-6 with
severe spinal cord compression at both levels
with malacia of the spinal cord.
Discectomy of C4-5 and C5-6 with removal
of the extrusion at the both corners of the field
until the dura was seen all over. Insertion of
OSIMPLANT EOS cervical bladed cages 13x15 H7 with IN'OSS
bone graft to C4-5 level and 13x15 6H to C5-6
level. Routine water-tight closure of the wound.
Smooth postoperative recovery.
The power of four limbs normalized. He was sent to the ward.
Comments
The recurrence in cervical discectomy is
zero. In rare cases when the patient progress OPLL, the
mentioned pathology could progress, masking the picture of
recurrence after long period of time.
