EXTRUDED DISC L5-S1 WITH LEFT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 27-October-2015
complaining of LBP for 12 years with
exacerbation and left sciatica for one month,
and numbness all toes left foot.
On examination: the patient was limping with
exaggerated scoliotic stance. SLRS was 80 degrees
with pain left side. Weak dorsi and planterflexion
left foot 3/5 with absent left AJ.
The patient was sent for investigations and MRI
done 29-October-2015 showed huge extruded disc
L5-S1 with left far downward migration. ESR was
55 mm/h and CRP was43 mg/mL for what Zinnat 500
twice daily was prescribed.
Foraminotomy left S1 root.
The left S1 root was severely compressed by the
extrusion The extrusion was removed lateral to
the axilla of S1 root.
Left sided intradiscal cleaning of L5-S1 disc
space. Using MultiGen, a pulsed mode
RF with 42 Celsius, 240 sec, 2 Hz and 20 mesec
duration the left S1 root was achieved.
Routine closure of the
wound.
Smooth postoperative recovery. The power of
the left foot became normal without any sciatica. The patient was
sent to the ward.

Comments
The patient still has an estimated
postoperative recurrence around 7%.
This is the 15th case using the PRF mode
with MultiGen.
