46 YEARS HUGE EXTRUDED DISC D12-L1 MORE TO THE LEFT.
Anamnesis
The patient came to the clinic 19-April-2014
complaining of LBP with left sciatica for 6
months, then bilateral the last months and now
more to the right. MRI lumbar spine done
15-March-2014 showing huge extruded disc D12-L1
with bulge D11-12, L3-4, L4-5 and L5-S1
On examination; the patient is limping with
exaggerated scoliotic stance. SLRS was 80
degrees both sides without pain. There is weak
dorsi and planterflexion both feet -4/5. Weak
also both quadriceps muscles 4/5. MRI of the
lumbar spine repeated the same day showing the
same data as before, but showing the extrusion
more to the left side.
Using the C-arm, the D12-L1 level identified.
Laminectomy lower 3/4th of D12 and upper 1/4th
of L1. Discectomy of D12-L1 disc was performed
from the left trying to avoid any traction
injury to the spinal cord. The centrally
extruded huge piece was removed in several
pieces until all the fragments were cleaned out.
Check of the right side for any extrusion was
negative.
Smooth postoperative recovery. The power of the
both legs became normal.
Comments
The patient still has a low postoperative
recurrence rate, less than 7%, because the area
is not under stress and the disc space height is
shallow.
