13-OCTOBER-2003 KHETAM SULAYMAN MARAR 50 YEARS SECOND RECURRENCE OF L5-S1 LEFT SIDE WITH
TOTAL COLLAPSE OF DISC SPACE.
Anamnesis
The patient was operated by me for fist
recurrence
06-April-2002. Then came to the clinic 12-October-2003
complaining of LBP with left sciatica with weak
dorsiflexion left foot.
MRI lumbar spine done 09-October-2003 showing
recurrent disc of L5-S1 left side.
On examination: the patient
is limping with
exaggerated scoliotic stance. SLRS
was 75 degrees left side with pain. There is weak
dorsiflexion left foot -4/5. There is hypalgesia
left L5
and S1 roots.
Neurolysis left S1 root. Foraminotomy
left S1 root. A huge extrusion was removed
lateral to the axilla.
Routine closure of the wound.
Smooth postoperative recovery. The power of the
left foot became normal and the left sciatica
disappeared.
Follow Up
The patient then came 21-October-2003 with clean
wound, without sciatica and neurologically free.
After then she disappeared.
Comments
The patient has this
second recurrence and a huge piece was removed.
In the previous surgery a huge amount of disc
material was removed and now the annulus
fibrosis seems to be the compressive factor.
It is very rare to have three surgeries for disc
prolapse, but here is an example.
After this the estimated postoperative
re-recurrence is zero because the disc space
collapsed completely.
