RIGHT CONVEXITAL CHRONIC SUBDURAL HEMATOMA WITH PENDING CONNING.
Anamnesis
The
patient came to the clinic in somnolence state
with complain of agonizing headache bifrontal
location for 2 weeks with projectile vomiting
with inability to set or walk.
On
examination: the patient had left sided paresis
with nuchal rigidity and inability to move the
neck to all direction.
The
patient was urgently sent for MRI of the brain
with MRA, which was done and confirmed the
presence of huge right convexital chronic
subdural hematoma with severe shifting of the
midstructures and the pons shifted down with
hydrocephalus. The patient was lucky, that the
tentorium was of wide variation.
The patient was urgently
taken to the operation room and 2 burr holes
were performed in the right frontal and parietal
region. Huge amount of hematoma came out, and
irrigation after inserting the external drain
was performed with saline.
Closure of the burr holes and
fixation of the external drain for 36 hours.

Comments
The patient was progressing conning during
visit. Urgent measures must be taken to prevent
the eminence of catastrophic squeals.
