11-SEPTEMBER-2014 LIQA MAHMOUD NOFEL 75 YEARS
SEVERE LUMBAR CANAL STENOSIS L3-4 AND L4-5 WITH BILATERAL DROP FEET.
Anamnesis
The patient came to the clinic 07-September-2014
complaining of LBP with left sciatica for 1
month with bilateral sciatica for 1 week with
numbness both lower limbs below the knees. MRI
done 04-September-2014 showing severe lumbar
canal stenosis L3-4 and L4-5.
On examination; the patient in wheelchair for 6
days with complete drop both feet and weak
planterflexion both feet 2/5. There is
hypalgesia below the knees both sides and there
is weak quadriceps muscles 3/5.
The patient was sent for more investigations.
Cervical and dorsal MRI showed small disci not
causing any spinal cord compression, not data
suggestive for transverse meylitis.
Decompressive laminectomy L3,4, lower half of L2
and upper half of L5. Foraminotomy L3,4,5 roots
both sides. All the compressive elements were
eliminated. Routine
closure of the wound.
Smooth postoperative recovery. The
planterflexion both feet became 5/5, but
dorsiflexion remain the same. Sensation improved
in both lower limbs.
Comments
The patient has complete drop both feet
within short period of time. But the drop feet looks old and
the sooner the intervention, the better the recovery.
