14-JULY-2002 MAHA JALAL BADDAR 41 YEARS
EXTRUDED DISC L5-S1 WITH RIGHT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic 06-July-2002
complaining of LBP for 3 months with right
sciatica. MRI
lumbar spine performed 06-May-2002 showing
huge extruded disc L5-S1 with right foraminal
occlusion.
On examination: The patient in agonizing pain,
limping with exaggerated scoliotic stance.
Positive cough sign. SLRS
70 degrees right side with pain. There is weak
dorsi and planterflexion right foot -4/5.
Right S1 foraminotomy with removal of the
extrusion lateral to the axilla. Right sided
intradiscal cleaning L5-S1. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak.
Routine closure of the wound.
Smooth postoperative recovery.
The power of the right foot improved.
She was sent to the ward.
Follow Up
The patient came to the clinic 27-August-2002
with clean wound and full power of the right
foot. SLRS was 75 degrees with pain.
The patient came to the clinic 19-April-2003
telling that the last 4 weeks got LBP for 3
days, then improved, then the last 5 days got
LBP with left sciatica. SLRS was 80 degrees with
pain in the left, but no motor, nor sensory
deficit. She was given medications and sent for
investigations, but disappeared.
Comments
There is still an estimated postoperative
recurrence around 7%.
The patient over the years will suffer
from other condition not related for his surgery.

fixation is the most accurate system in the market. The microdrive and
its sensor gives feed back about the localization.
