WITH RIGHT DOWNWARD MIGRATION.
Anamnesis
The patient was operated by me
14-December-2006. The patient showed
postoperative improvement of the power of the
right foot.
The patient then came 29-April-2007 with
exacerbation of the LBP with no sciatica. SLRS
was 45 degrees in the right with no neurologic
deficit. Check MRI performed 24-May-2007 showing
a huge recurrence at the same level of L4-5 with
right downward migration.
The patient escaped and reappeared
23-October-2007 with deterioration of his
condition with hypalgesia of the right S1 root
and weak dorsi and planterflexion right foot.
The patient was sent to another MRI, which done
30-October-2007 and confirmed the presence of
the same extrusion seen at 24-May-2007.
The right L5 root was exposed from the upper
corner and followed down, dissecting during that
the dense scar. Piece-meal removal of the
extrusion lateral to the axilla. A huge amount
of extrusion was removed and meticulous cleaning
of the disc space from the right was performed.
Smooth postoperative recovery with normalization
of the power of the right foot.
Comments
Reviewing the previous surgery, it was
believed that the patient mostly will catch a recurrence of
his disc.
This situation, can be avoided by
installing some material inside the disc space after
performing traction to avoid slippage of the inserted
material. But, any foreign body have its disadvantages.
In the future, if such material could
stand practice will be introduced in the protocols in such
situations.
