62 YEARS CALCIFIED LESION COMPRESSING C4-5 LEFT SIDE WITH QUADRIPARESIS.
Anamnesis
The patient came to the clinic
13-October-2003 complaining of LBP for 20
years and was operated 1982 for PLD L4-5
and 1992 for recurrence. He is using crutches
for walking for 1 year with neck pain with
numbness ulnar division both upper limbs for 6
months. MRI cervical spine done 10-October-2003
showing severe compression by a solid mass at
the level of C2-3 from behind the left side with
malacia of the spinal cord.
On examination: the patient has severe neck pain
when rotating the head to all directions. There
is deep quadriparesis and quadrihypalgesia below
C3 with exaggerated deep reflexes with
pathologic reflexes.
Decompressive laminectomy
C2-3 with removal of the mass from the left side
until the dura was seen free entirely from above
and below the lesion. It was sent for biopsy.
Routine closure of the wound.
Smooth postoperative recovery. The power of the
four limbs moderately improved. He was sent
to the ward.
FOLLOW UP
The patient
came to the clinic 26-October-2003 with clean wound
with full power of the right side and still having
weak extension left hand 4/5 and left quadriceps
femoris. The patient told to come after three
months, but he disappeared.
Comments
The histologic result was ligamentum flavum
hypertrophy.
