YEARS EXTRUDED DISC L4-5 WITH LEFT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic 15-March-2014
complaining of LBP for 9 months with
exacerbation of LBP and left agonizing sciatica
for 2 months. During this period he cannot set
down due to the agonizing pain. MRI lumbar spine
done 03-March-2014 showing extruded disc L4-5
with left foraminal occlusion and downward
migration.
On examination; the patient is limping with
exaggerated scoliotic stance. SLRS was 80
degrees right side and 30 degrees with pain in
the left. There is weak dorsiflexion left foot
3/5 and planterflexion same foot 4/5 with
hypalgesia left L5 and S1 territories.
Using C-arm, the L4-5 level was identified.
Foraminotomy left L5 root with removal of the
extrusion lateral to the axilla. Left sided
intradiscal cleaning L4-5 disc space. Routine
closure of the wound.
Smooth postoperative
recovery. The power of the left foot became
normal.
Comments
The patient still has an estimated
postoperative recurrence rate around 7%, because
the disc space height still not shallow.
