HUGE PITUITARY ADENOMA PUSHING THE OPTIC NERVES AND MAXIMAL INFRASELLAR GROWTH.
Anamnesis
The patient came to the clinic 31-October-2004
complaining of bifrontal headache with decreased
vision right eye for 4 months with progressive
course. MRI of the sella performed
25-October-2004 showing giant pituitary adenoma
compressing the optic chasm and optic nerves
more the right with huge infrasellar extension.
On examination: the patient has decreased vision
right eye but the OMNs are intact. The patient
was sent for fundoscopy, VA and perimetery and
hormonal studies.
Trans-sphenoidal approach was applied and
radical resection of the tumor was achieved and
proven by fulfilling the area with contrast
agent and the cardio-pulmonary pulsation at the
chiasmal cistern was proven by the C-arm. No CSF
leak and the nasal cavities were filled with
special gauzes with tubes.
Smooth postoperative recovery and the patient
was sent to the ward.
Follow Up
The patient came to the clinic 06-February-2005
with new MRI showing complete radical resection
of the tumor. Vision normalized. In no
replacement or hormonal therapy. He was advised
to undergo radiotherapy.
Comments
The patient has the massive infrasellar
extension, appealing the neurosurgeon to go
trans-spheniodally.
