27-AUGUST-2014 MAHMOUD BASSAM KHALEEL 8 YEARS
PERSISTENT CSF LEAK FROM GROWING FRACTURE OF THE RIGHT SUBSTANCIA CREBROSA.
Anamnesis
The patient came to the clinic 23-August-2014
complaining of CSF leak from the right nostril for
4 years with several attacks of meningitis. He
sustained RTA with fracture base of the skull
when he was 1 year old. CT-scan 0f the skull
base done 16-August-2014 showing bony defect of
the right substancia crebrosa about 6 mm defect
with the CSF filling the right ethmoid sinuses.
MRI done the same day confirming these data.
On examination; the patient has right sided
anosmia. Otherwise he is neurologically free.
Bifrontal approach with reflection of the bone
flap to the right. Extradural approach to the
damaged right olfactory bulb. The dura was
dissected around the bone defect, which was 15x5
mm in dimensions. The other olfactory bulb and
crista galli were not violated in dissection.
Using 6 zero nylon, water-tight closure of the
dura and aided with piece of muscle filled to
the bone defect area. Routine
closure of the wound.
Smooth postoperative recovery.
Comments
The patient had during trauma a fissure
fracture, which over time got bigger. Only surgical repair
can resolve the CSF leak and prevent further attacks of
meningitis.
