YEARS EXTRUDED DISC L5-S1 WITH RIGHT FAR DOWNWARD MIGRATION AND RIGHT L4-5
LATERAL RECESS SYNDROME.
Anamnesis
The patient came to the clinic 07-October-2015
complaining of LBP for 4 months without
sciatica. MRI done
06-August-2015 showed huge extruded disc L5-S1
with right far downward migration. During the
first 2 months there was numbness right foot,
which resolved.
On examination: the patient was not limping with
no scoliotic stance. SLRS was 30 degrees
with pain right side. Weak dorsi and planterflexion
right foot 4/5.
Foraminotomy right L5, S1 and
S2 roots.
The right S2 root was severely compressed by the
extrusion The extrusion was removed subaxillary
under the S1 lateral to the right S2 root.
Inspection of the annulus fibrosis of L4-5 and
L5-S1 showed no proper extrusion, for what these
disc spaces were not violated. Using MultiGen, a pulsed mode
RF with 42 Celsius, 240 sec, 2 Hz and 20 mesec
duration the right L5, S1 and S2 roots was achieved.
Routine closure of the
wound.
Smooth postoperative recovery. The power of
the right foot became normal. The patient was
sent to the ward.

Comments
The patient still has an estimated
postoperative recurrence around 7%.
This is the 14th case using the PRF mode
with MultiGen.
