22-MARCH-2013 MAJIDA JAWAD TMEZSE 40 YEARS
HUGE EXTRUDED DISC L5-S1 WITH RIGHT FORAMINAL OCCLUSION.
Anamnesis
The patient came
to the clinic 16-March-2013 complaining of LBP
for 4 months with right sciatica for 2 months
down to the lateral malleolus right ankle.
MRI lumbar spine done 16-March-2013 showing
huge extruded disc L4-5 with right foraminal
occlusion.
On examination: The patient is limping with
exaggerated scoliotic stance. SLRS was 50
degrees in the right with pain and 80 degrees in the left.
There is almost drop right foot 2/5 and severe weak
planterflexion 3/5 same foot with hypalgesia
right S1 root.
Using C-arm, the level of
L5-S1 was identified, because there was
lumbarization of S1. Foraminotomy right S1 root
was done. The
extruded extrusion was
removed lateral to the axilla in several pieces. Right sided
intradiscal cleaning L5-S1 disc space.
Routine closure of the wound.
Smooth postoperative recovery. The power of
the right foot became normal.
Comments
There is still an estimated postoperative
recurrence around 7%, because the disc space is
still not shallow.
