22-JUNE-2014 MALIK MUHAMED AL-MOMANI 40 YEARS
EXTRUDED DISC L2-3 WITH UPWARD MIGRATION AND SEVERE SEGMENTAL STENOSIS.
Anamnesis
The patient came to the clinic 21-June-2014
complaining of LBP for 2 months after falling
down with left
sciatica for 45 days down to the left knee. The
patient had RTA 10 years ago. MRI lumbar spine
done 15-May-2014 showing extruded disc L2-3 with
upward migration with severe stenosis at this
level.
On examination; The patient is
limping with exaggerated scoliotic stance. SLRS
80 degrees with pain in the left. There is weak
quadriceps muscle right 3/5, left 2/5, abduction
and adduction right knee 3/5, dorsiflexion both
feet -3/5.
The patient was sent for investigations and MRI
lumbar spine done the same day showing the same
extrusion with severe segmental stenosis.
Using C-arm, the L2-3 level was identified.
Laminectomy of lower 2/3 of L2 and upper third of L3 was
achieved. Intradiscal decompression was achieved
from both sides and the extruded disc was
removed from both sides. Further intradiscal
cleaning of L2-3 disc space. Foraminotomy was
not performed, since the extruded disc was
mainly upward migrating.
Smooth postoperative recovery. The power of
both lower limbs became better.
Comments
The patient still has an estimated
postoperative recurrence around 7%, because the disc space
is still not completely shallow.
