The patient a Sudani citizen came to the hospital complaining for several months  of weak both lower limbs with hypalgesia of the perianal region and down from both knees with difficult micturition and defecation. The condition is progressing.

MRI of the lumbar spine performed recently showing a mass behind L1-2 compressing the spinal cord and the running roots.

Laminectomy of L1-L2 was performed over the mass and the dura was opened and radical resection of the schwannoma was achieved. The patient was put in Reverse Trendelenburg position with Valsalva maneuver and hyperventilation. No CSF leak. Routine closure of the wound.

Smooth postoperative recovery. The power of the feet improved. He was sent to the ward.

The patient was seen in the clinic 06-March-2002  with clean wound and normal power of both legs and normal micturition and defecation. He was advised to repeat MRI of the spine after 6 months, but he disappeared.

The patient had schwannoma of the conus medullaris, causing weakness both legs and cauda equina syndrome, which resolved after surgery.
