04-DECEMBER-2007 MANAL KAMEL QASEM 37 YEARS HUGE EXTRUDED DISC L4-5 WITH LEFT
DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 01-December-2007
complaining of LBP for 10 years with left
sciatica. MRI done 31-December-2002 showed bulge
L4-5. The patient got exacerbation of her left
sciatica the last 10 days. MRI performed
27-November-2007 showing huge extrusion of L4-5
with left downward migration.
On examination: the patient is limping for one
week with exaggerated scoliotic stance. SLRS was
20 degrees in the left with pain. She had almost
drop left foot with weak planterflexion left
foot 4/5 and hypalgesia left L5 and S1
territories.
Left L4-5 hemiflavotomy with foraminotomy of
left L5 root was performed and the extrusion was
removed lateral to the axilla. Meticulous
cleaning of the disc space was done from the
left side. The epidural fat was transferred to
cover the left L5 root. Routine closure of the
wound.
Smooth postoperative recovery.
Comments
The patient is very obese and other
hospital refused to operate her due to that reason. Obesity
is not contraindication for surgery and they must be treated
as usual. She showed prompt postoperative improvement.
