15-SEPTEMBER-2025  MANAR ISMAEEL  BADAWI 28 YEARS  FLAT LEFT FOOT WITH TARSAL TUNNEL SYNDROME.

 

Anamnesis

The patient came to the clinic to Dr, Ali Al-Bayyati 12-September-2020 complaining of chronic pain and burning sensation of the left heel and foot, especially after prolonged standing and walking and not responding to medical treatment.

On examination, the patient is limping due to left foot pain. Tinel's sign positive over the left posterior tibial nerve. Flat both feet more the left. The hindfoot in valgus  and the forefoot abducted.

The patient was sent for investigations and X-ray of the foot in AP and lateral position and EMG of both posterior tibial nerves.

The EMG confirmed severe left tarsal tunnel syndrome.

Under G.A, with tourniquet to the left foreleg, 2 separate incisions were done over the projection of the left posterior tibial nerve. In the above incision the posterior tibial nerve was found and neurolysis was achieved, releasing during that the edge of the tarsal tunnel was bisected and through the lower incision the three major divisions of the nerve including the Baxter nerve were identified, released. Reconstruction of the medial arch by half transfer of the tibialis anterior into navicular  (Young's tenosuspension) and tibialis posterior advancement (Kinder procedure) were achieved. Tenodesis of the tibialis anterior and posterior was done.  Routine closure of the wounds after strict hemostasis. She was sent to the ward.
